How to register

Provided you are residing within our practice area, you may register at any of our surgeries by completing a registration form. Alternatively you can download a registration form, using the following links, but this cannot be
submitted online, it must be completed and signed by the patient or the patient’s representative and handed in at reception.

Facilities

Access for Wheelchair users. All sites have level access.

Haxby has automatic doors.

New Earswick please ring bell for access assistance if needed.

Huntington please ring bell for access assistance via a side door.

Stockton-on-the-Forest please ring bell for access assistance via a ramp.

Parking - Huntington has limited parking, please park away from the site leaving parking spaces for doctors and the disabled.

Protection and Use of Patient Information

Your records are kept in a secure and confidential manner and use is restricted to the terms of the Data Protection Act (1998).

We have an obligation to record relevant clinical details about our patients. This information is mainly recorded on the practice computer system. Presently these computer records are required to be kept indefinitely. They are available on a need to know basis to clinicians and administrative staff in the practice. The information in the patient records is used to facilitate ongoing care, for recall and for other administrative purposes such as repeat prescriptions. We do use clinical information for audit and research (including market research) but this is in an anonymised form with no patient identifiable details.

We have strict protocols for access and confidentiality. Information from the clinical record may be released to third parties when a referral is made to a hospital - for instance to a specialist - and on other occasions such as for a report to a solicitor or insurance company. In the example of a specialist referral, consent for release of relevant clinical details is assumed as it is essential information without which a referral cannot be made. We do not release clinical details to other third parties without explicit consent from the patient. We cannot discuss patient's details with friends or relative without the patient's consent.

As a patient you are entitled to view your clinical record and amend inaccuracies if found. There is normally a administrative charge for this service.

Patients Advice and Liaison Service (PALS)

NHS PALS can aid you with queries regarding patient’s rights and NHS services. They can also help you in the event of a lack of satisfaction with the outcome of a complaint to the Practice.

In addition, the Practice has its own member of staff who can advise regarding Practice services or who can assist with general suggestions, compliments or complaints. Phone 724600 and ask for the Practice PALS coordinator.

Your rights and responsibilities are shown on the Department of Health Website. The booklet “Your Guide to the NHS” also contains details and copies are available from any surgery reception.
